Pre-Event Questionnaire

To best meet your groups needs, Val wants to understand the issues your organization deals with and the goals you have for this meeting. This pre-event questionnaire helps her customize her presentation to better relate to your particular group.

The questionnaire is a PDF form; in most browsers, clicking on the thumbnail to the right will cause it to open in a new window. After it opens, you will be able to fill in the fields and then click the "submit" button to e-mail the form to us.

If you'd like to download the form and fill it out later, right-click on the thumbnail and choose "Save Target As." Once saved on your computer, you can open and edit it at any time, even if you are not connected to the Internet.
